Virtual Follow-up

Service Description

Follow-up virtual appointments allow a convenient way continue physical therapy treatment, pain and headache management, or performance fitness sessions. Please be sure to have adequate internet connection with video and audio available for the duration of your session, along with appropriate space and attire to move.

Cancellation Policy

We appreciate our clients and patients who choose Functional Health Consultants, PLLC for their skilled physical therapy services. Patient are highly encouraged to adhere to service recommendations and attend scheduled appointments. As our time is limited, we encourage our clients and patients to keep appointments. Cancellation or rescheduling should occur prior to 24 hours of appointment times. A 50% retainer fee will be charged for cancelation or rescheduling within 24 hours of the scheduled appointment time. When rescheduling occurs 3 times during a case the option to return to the cue for scheduling will require a non-refundable retainer fee at a rate of $50 per 15 minutes.
