Physical Therapy Consultation
Service Description
Initial comprehensive physical therapy consultation for in-person services allows our therapists to assess patients and initiate programs to assist in achieving their goals through a variety of treatment options, including hands on therapy. This consultation can be used for physical therapy treatment, pain and headache management, strength training, and performance fitness sessions.
Cancellation Policy
Cancellation, Rescheduling & Attendance Policy At Functional Health Consultants, PLLC, we value your time and our commitment to your care. To ensure fairness and availability for all clients, we maintain the following policies for both in-person and virtual physical therapy sessions. Cancellations & Rescheduling Appointments may be canceled or rescheduled without penalty if notice is given at least 24 hours in advance. Cancellations or reschedules made within 24 hours of your appointment time will incur a late cancellation fee equal to 50% of the session cost. No-shows (missed appointments without notice) will be charged the same 50% fee as a late cancellation. All fees are non-refundable and do not apply to future sessions. Repeated Rescheduling If an appointment is rescheduled three times during a single treatment plan, any further bookings will require a non-refundable scheduling fee of $50 per 15 minutes reserved. This fee will be applied toward the cost of the session but is forfeited if the session is missed or canceled late. Late Arrivals We allow a 10-minute grace period for late arrivals. After 10 minutes, your session may be shortened to accommodate the schedule or may need to be rescheduled. If you anticipate being late, please contact us as soon as possible. While we strive to be flexible, we cannot guarantee full session time or availability for rescheduling on short notice. Emergencies & Exceptions We understand that genuine emergencies happen. In rare cases, we may waive late cancellation or no-show fees at our discretion. However, repeated requests for exceptions or patterns of last-minute cancellations may result in fees being applied regardless of the reason, in order to maintain fairness to other clients and protect our time.
