In-Person Consultation

Service Description

Initial physical therapy consultation for in-person services allows our therapists to assess patients and initiate programs to assist in achieving their goals through a variety of treatment options, including hands on therapy. This consultation can be used for physical therapy treatment, pain and headache management, strength training, and performance fitness sessions.

Cancellation Policy

We appreciate our clients and patients who choose Functional Health Consultants, PLLC for their skilled physical therapy services. Patient are highly encouraged to adhere to service recommendations and attend scheduled appointments. As our time is limited, we encourage our clients and patients to keep appointments. Cancellation or rescheduling should occur prior to 24 hours of appointment times. A 50% retainer fee will be charged for cancelation or rescheduling within 24 hours of the scheduled appointment time. When rescheduling occurs 3 times during a case the option to return to the cue for scheduling will require a non-refundable retainer fee at a rate of $50 per 15 minutes.
